On April 11 th and 12 th the Federation for Children with Special Needs, Holyoke Community College, Abilities Unlimited and the Multicultural Community Services will be conducting a 2-day transition planning conference focusing on students with disabilities moving from school to adulthood.

• Do you have a child between the ages of 15 to 22?
• Have you thought about …….
  -what they will do after high school or after turning 22?
  -post-high school options like inclusive concurrent enrollment?
  -where will they work or if they will work and how will they get there?
  -how your child will spend their days?
  -where they will live and who will support them?
• Does your child have the skills needed to be independent?
• What do you need to know as a parent/guardian to assist your young adult to become self reliant, independent, employable, and happy as they can possibly be?
• How do you, as a parent/guardian, fade your involvement to help teach your young adult to speak up for what they need and find people to help them?
• What should you be doing now to make the most of high school opportunity and for the transition to the adult community?
